D1: Manage the health and wellbeing of individuals, groups and communities, focusing on health promotion, disease prevention and health protection

D2: Promote health and wellbeing through education, awareness and advocacy, with a focus on health inequalities and social determinants

D3: Assess and support individuals with complex health and social care needs, including those with long-term conditions and disabilities

D4: Develop and implement care plans that promote the health, wellbeing and independence of individuals, with a focus on person-centred care

D5: Manage and lead health and social care teams, including nurses, care workers and other support staff, to deliver high-quality care and services

D6: Analyse and evaluate the effectiveness of health and social care services, using data and evidence to inform practice and policy

D7: Develop and implement policies and procedures to promote health and wellbeing, with a focus on health and safety, risk management and quality assurance
